I also noticed there wasn't a question on Joseph translating with his head in a hat.
I don't know any Chapel Mormons that are aware of it.

A few thoughts I had when reading the questionare for the first time:

I would remove question #15.
The birth control issue doesn't really apply anymore and I'm not sure why that would define Chapel vs. Internet. Except for the McKonkie type Chapel Mormons, both groups have let those former teachings slip away (the church doesn't intrude in the bedrooms much these days) and use personal revelation to determine the size of their family. And of course both groups believe anything the modern day Prophet says (including their silence on previous teachings/doctrine) Trump's everything so the old teachings on it don't matter to them. I would say they are more concerned with permanent sterilization, specifically the male vasectomy since that is in the current church handbook as taboo.


in my opinion, you might want to change the wording on #6, 16 and 20 to "spiritually dangerous" or something like unfaithful/disobedient to God. The first time I read those, it sounded extreme.
