IHQ is quoting from the current handbook, which was issued within the last few years. I think you and I were out long before there were any such guidelines.
Res Ipsa, I first followed the link and read down finding nothing like the quote, hence my puzzle. Because of your observation I tried again only scrolled up a ways to find the quote and subject. It was there. Ok.
