Sorry if I am asking for a historical fact that may be known to you all, but it is not to me.
I know that we talk about the PoGP having been canonized twice by acclamation at general conferences. The LoF perhaps having been included in the D&C without canonical approval from the general membership, and thus taking out by fiat of the Brethren in the early 20th Century. And of course, there was the precursor to the D&C, the Book of Commandments, once part of the canon but updated and renamed the D&C--to which the general conference assemblies have approved additions from time to time.
But when was the Book of Mormon voted upon? The Bible? Have any of the numerous changes to the Book of Mormon been put to vote? Have provisions of the Book of Mormon, like the trinitarian notion of the godhead and the prohibition against polygamy except when needed for a 'righteous seed', been put up for a vote for removal (but perhaps been voted down)?
Has there been any attempt to reconcile the inconsistency between Jacob 2 (Book of Mormon) in which it clearly states that David's and Solomon's concubines were an abomination to god, and D&C 132:1 wherein it is claimed that but for the incident of Uriah's wife, David and Solomon were justified by having those 'abomination' concubines?
At which general conference(s) were Bible & Book of Mormon canonized?

1880 was the most notable conference where a vote was taken for the D&C and Pearl of Great Price.
There were other votes; the famous 1835 vote when Joseph Smith wasn't present and the article of marriage was introduced.
Votes for OD1 and OD2.
In my lifetime I recall a vote as to the 1978 or 1979 version of the quad.

Maybe. I am of the view that "canonization" isn't a real concept the Church follows; rather, a decision is made by the Brethren at some point as to what is or is not published in the LDS Scriptural "quad," which includes such things as the Topical Guide and the Bible Dictionary.
When I was on my mission, for example, the Bible Dictionary was published by Oxford, and was basically an Anglican work. The New and Old Testament had the marginal readings included, which are a part of the official KJV.
The 1978-1981 McConkie revisions modified the Bible Dictionary to make it more consistent with LDS Doctrine. The marginal readings were dropped, which made our KJV not consistent with the Authorized Version. Some of those marginal readings are quite supportive of LDS Doctrine.
The 2013 changes appear, as to the Bible Dictionary, to back away from some more of the folksy McConkie views of the fall, the godhead and such, although I haven't see a full assessment of that.
I don't think a vote is necessary for this, but there was a precedent established by Joseph Smith as early as 1830.

I don't recall, but there was a vote to approve the publication of the Book of Commandments.

I would agree with that, only because no revelation requires it. However, in 1880 Orson Pratt or somebody like him held up the Doctrine and Covenants and Pearl of Great Price and asked the conference attenders to raise their right hand to affirm that these were scripture.
By contrast, revelation does require church affirmation for callings. One of my favorite Mountain Meadows Massacre stories involved one of the perpetrators, William Dame, a Stake President who was one of the commanders of the militia. Brigham Young went to his stake to release him after the massacre. The stake would not give a majority vote to give a vote of thanks to him or sustain the new SP. President Young angrily castigated the Saints and told them that they'd have to live with Dame, then, for another 10 years. The Lord punished Dame, apparently. No children by his four wives. The name Dame is not known as an LDS name because he had no descendants.
