That's a pretty good description of my previous experience. Sadly, I suspect we've lost access to the moderator section of the board, which provided pretty good training just by reading the interactions between moderators and Shades over the years. But Shades was always willing to communicate regarding how he wanted moderators to make and implement decisions.

Multiple uses or use in a sexual context generally draws an R. But there are examples of movies with many f-bombs that are rated below R. It's pretty loosey goosey.
