How would the women and men know? Aren't Mormon initiates supposed to be in some hypnotic state? I apologize to Shades in advance if this question is wrong. Otherwise, wouldn't initiates yell out something about hands-off?

You're missing the point. It was a temple ritual. The person administering the initiatory had no choice OTHER THAN to touch the genitals, similar to how a person performing a baptism has no choice OTHER THAN to immerse the person completely.
When genitals stopped being touched, it was only because the higher-ups officially changed the way the ordinance was performed.
I sure hope there weren't any instances of a sixth point of fellowship.
