If you have an LDS membership number you are able to see the date (typically exact) and the temple that the proxy work was performed at.
You may be able to see who has submitted the name. Often there is more than one person, though. The capability to discover the actual proxy would be hit or miss. Older work, from the 19th Century might show you the proxy on the microfilm of the temple records. Those microfilms reside in the FHL in the Special Collections room and can only be viewed by TR holders (or those with a letter from their bishop).
I do not believe that the names the temple hands out have a way of keeping track of the proxy.
